Role Description
As a Dietitian, you will play a crucial role in supporting patients with one of the largest health plans in the United States. Your responsibilities include answering patients' questions, helping patients onboard/offboard, assisting patients in improving and maximizing health outcomes, and establishing supportive relationships with patients. This role cares about our patients and aims to deliver an exceptional experience.

Job Description
- Facilitates chronic condition management programs using virtual communication tools such as Zoom or phone.
- Programs include Diabetes Self-Management, Diabetes Prevention, and Cardiovascular Disease, incorporating clinical, educational, and health coaching components.
- Develops individualized interventions to improve health outcomes.
- Identifies and enrolls individuals eligible for chronic condition management programs.
- Builds supportive and confidential relationships with patients.
- Collaborates with team members including program directors, case managers, Medical Doctors, and Health Coaches to optimize care.
- Proficient with technology: Apps, Slack, Google Calendar, Calendly, Zoom.
- Conducts outreach via phone, email, and mail.
- Provides face-to-face and telephonic health coaching in individual and group settings.
- Utilizes critical thinking and problem-solving skills in condition management.
- Performs other duties as assigned.
Qualifications
- Registered Dietitian Nutritionist (RDN). Licensure in New York required if not already obtained.
- Fluent in Spanish and English.
- Proven ability to multitask effectively.
- Excellent critical thinking skills.
- Experience in leading presentations; energetic personal style; process-oriented thinking.
- Passion for health and wellness.
- Proficiency with relevant technology and software.
