Position Overview
As a Continuous Care Coach, you’ll provide personalized, high-quality care coordination for patients with chronic eye diseases. You will work closely with patients, their families, healthcare providers, and our internal teams to create and implement tailored care plans. Top candidates will thrive in a remote environment, engaging with up to 15 patients daily while demonstrating empathy, professionalism, and a passion for patient care.



Job Type: Full-time


Job Location: Remote


Pay: $20.00 - $23.00 per hour


Responsibilities

Communicate with patients, families, and caregivers to provide education and guidance on managing chronic eye conditions.

Collaborate with home health agencies and community services.

Collect and document health outcomes and registry data.

Assist with treatment adherence, medication management, and access to care.

Develop, maintain, and communicate personalized care plans.


Requirements

Active Certification: COA, COT, or COMT required.

2+ years of experience in eye care or healthcare operations.

Proficiency in EMR systems preferred.

Exceptional communication and organizational skills.

Ability to work remotely in a HIPAA-compliant workspace with reliable internet access (minimum 50 Mbps download/10 Mbps upload).
