Job summary

Alignment Health is seeking a Complex Care RN to serve as clinical quarterback for high-risk Medicare Advantage members, coordinating care virtually and in collaboration with a multidisciplinary team.

You will manage transitions of care from hospital or SNF, perform medication reconciliations, monitor status changes, and contribute to HEDIS and care gap closure while documenting in Athena EMR. Bilingual Spanish/English required; CA RN license essential.

Qualifications

• RN with 3+ years in complex care, care management, transitions of care or related clinical settings.
• Bilingual in Spanish and English (read, write, speak).
• Experience with Medicare Advantage, chronic disease management and care transition coordination.

Responsibilities

• Coordinate care for medically complex Medicare Advantage members in virtual and in-person settings.
• Lead transitions of care following hospital or SNF discharges and perform medication reconciliation.
• Identify clinical concerns and escalate to appropriate providers; document in Athena EMR.
• Support HEDIS and care gap closure activities with multidisciplinary teams.
