Job summary

Blue Cross and Blue Shield of Kansas City seeks a Clinical Social Worker to assess needs, plan care, and advocate for members in the Kansas City metro area. You will collaborate with the care team to coordinate resources, support case and disease management, and promote cost-effective outcomes.

A license in MO or KS is required with secondary license within 30 days. Ideal candidates hold a MSW, experience in medical social work, and strong communication.

Qualifications

• Bachelor's degree in social work and a social work license in MO or KS.
• 3–5 years of health care setting experience; hospice/oncology/palliative preferred.
• Ability to read, write, analyze, and interpret complex information.
• Must reside in the Kansas City metropolitan area.

Responsibilities

• Complete member assessments across seven health domains and develop individualized care plans.
• Coordinate with care team to monitor plans and support end-of-life care when needed.
• Educate members and families; adapt teaching to health literacy levels.
• Meet program targets, manage caseloads, and document activities thoroughly.
