Job summary

Ennoble Care is seeking a per diem Hospice Social Worker in the Kansas City, KS area. You will perform psychosocial assessments, participate in IDG meetings, and help develop the patient’s plan of care. The role requires strong written and verbal communication, confidentiality, and a valid driver’s license.

The position offers per-visit and per-admission compensation with mileage reimbursement, and includes full-time-style benefits for eligible employees.

Qualifications

• Master's in Social Work (LMSW) required.
• Hospice experience preferred.
• Must be a licensed driver with reliable vehicle.
• Strong communication skills, both written and verbal.

Responsibilities

• Conduct psychosocial assessments within 5 days of admission.
• Participate in IDG meetings and care planning.
• Observe and report changes in emotional and social factors.
• Maintain and submit clinical records including evaluation and care plans.
• Coordinate visits with Case Managers weekly.
