Job summary

The Mount Sinai Health System is seeking a Health System Command Center Clinical Pharmacy Manager to provide centralized clinical pharmacy services via a virtual command center. This role supports inpatient, hospital‑at‑home, and ambulatory care environments with remote medication order verification, reconciliation, and telepharmacy services.

Responsibilities include collaboration with care teams, drug safety, and care coordination across multiple sites.

Qualifications

• Active NY pharmacist license
• PharmD or BS Pharmacy degree
• PGY1 residency preferred
• PGY2 residency preferred
• Experience with Epic / CPOE systems, remote order verification, telepharmacy, medication reconciliation

Responsibilities

• Perform centralized verification of medication orders across multiple hospitals.
• Support Hospital at Home medication management and telepharmacy.
• Coordinate with physicians and nurses to resolve medication‑related issues.
• Provide discharge medication reconciliation and post‑discharge follow‑up.
• Monitor medication safety and ensure protocol compliance.
