Qualifications

Active pharmacist license in New York state.
PGY1 residency preferred; PGY2 residency preferred.
2–5 years hospital or health-system pharmacy experience.
Experience with Epic / CPOE systems, remote order verification, telepharmacy, and medication reconciliation.

Responsibilities

Perform centralized verification of medication orders across multiple hospitals and care sites.
Review dosing accuracy, drug interactions, allergies, contraindications, and formulary compliance.
Collaborate with physicians, nurses, and pharmacy teams to resolve medication-related issues.
Provide telepharmacy consultation and real-time remote medication verification.
Support Hospital-at-Home programs with medication reconciliation and orders.
Coordinate with site pharmacists to ensure continuity of therapy across care settings.
