Job summary

Devoted Health is seeking a pharmacist with 3+ years of direct patient care experience to join a high-performing team. The role focuses on pharmacist consult services, medication therapy management, and improving clinical outcomes through personalized care plans.

The position supports remote work with Pacific or Central hours and requires multi-state licensure and language skills. You will collaborate with providers, guide patients through benefits, and help drive 5-Star pharmacy performance

Qualifications

3+ years of clinical experience in direct patient care.
State licensure in good standing in multiple states; at least 5 states required.
Residence in Mountain, Pacific, or Hawaiian time zones or ability to support clinic hours in those zones.
Fluent proficiency in Cantonese/Mandarin.
Experience with collaborative pharmacy practice agreements and geriatric care is a plus.

Responsibilities

Provide pharmacist consult services to manage chronic conditions with personalized care plans.
Conduct comprehensive Medication Therapy Management reviews for complex regimens.
Drive quality programs and meet 5-Star pharmacy clinical measures.
Mentor team members and support clinical tooling and workflows, including AI-enabled systems.
