Job Title: Remote Clinical Manager

Pay: $86,000-$96,000 per year

Responsible for providing remote or on-site Clinical Manager coverage in care centers under the direction of the AVP Clinical. Coordinates with and takes direction from Directors of Operations of assigned care centers. Ensures the delivery of quality patient care while demonstrating fiscal responsibility and maintaining high standards of care, compliance with policies, procedures, and regulatory requirements. Accountable for monitoring and managing the quality of patient care, achieving key performance indicators, and organizational clinical performance.

Responsibilities

1. Oversees scheduling to meet patient needs, reviewing assignments with staff as appropriate.
2. Manages payor verification and pre-certification, communicating with payors and responding to information requests.
3. Reviews clinician documentation for accuracy and completeness, including Oasis data, care starts, recertifications, and physician orders.
4. Provides feedback to clinicians on documentation, skills, and behavior.
5. Assists clinicians in establishing patient goals and developing the Plan of Care, overseeing its implementation and review.
6. Ensures timely communication of patient condition changes and necessary adjustments to the physician and staff.
7. Evaluates patient needs and revises the care plan accordingly.
8. Leads patient care conferences and promotes effective communication among care teams.
9. May provide direct patient care occasionally in emergencies.
10. Participates in after-hours on-call processes as needed.
11. Engages in quality assurance activities, including chart audits and data collection.
12. Responds to documentation requests and denials as directed.
13. Guides clinicians to deliver quality home care services and provides support during operating hours.
14. Coordinates activities across disciplines to ensure service quality and continuity.
15. Handles patient, family, and physician concerns and investigations.
16. Conducts training for new Clinical Managers and mentors staff.
17. Recommends employment decisions, including hiring, firing, and promotions.
18. Performs additional duties as assigned.

Qualifications

• Associate or Bachelor's degree in Nursing.
• Current, unencumbered RN license in the state(s) of work, possibly including compact licenses.
• At least 1+ year of supervisory/management experience as a Registered Nurse.
• At least 1+ year of RN experience in Home Health or Hospice.
• Current CPR certification.
