Job summary

Sana Lake Recovery, LLC is seeking a Clinical Chart Auditor to support our substance use and behavioral health programs in Missouri. This full-time role focuses on auditing clinical and medical records across levels of care to ensure completeness, accuracy, and regulatory compliance.

Candidates should have an associate degree and at least one year of behavioral health experience, familiarity with EHRs, and knowledge of ASAM criteria and HIPAA.

Qualifications

• Associate degree in a relevant field, preferred.
• Minimum of one year of experience in behavioral health or substance use disorder.
• Experience with an electronic health record system; preferred.
• Familiarity with ASAM Criteria, Missouri Department of Mental Health requirements, Joint Commission standards, HIPAA, 42 CFR Part 2, and payer documentation expectations.

Responsibilities

• Conduct routine and focused audits of client records across applicable levels of care, including detoxification, residential, PHP, IOP, outpatient, and recovery-support services.
• Review documentation completed by counselors, therapists, case managers, nurses, physicians, nurse practitioners, physician assistants, and other members of the treatment team.
• Evaluate records for completeness, accuracy, consistency, timeliness, appropriate authentication, and compliance with organizational policies.
• Confirm that documentation accurately reflects the services provided, the client’s clinical condition, progress toward treatment goals, and ongoing need for the current level of care.
• Review admission assessments, biopsychosocial assessments, medical evaluations, treatment plans, progress notes, group notes, individual and family therapy notes, case-management documentation, medication records, continued‑stay reviews, transition plans, and discharge summaries
