Job summary

Vitasora Health is seeking a remote Care Coordinator to join our Chronic Care Management and RPM programs. You will conduct monthly outreach, apply health coaching principles, and develop CCM care plans centered on patient preferences and outcomes.

You will collaborate with billing, clinical leadership, and referring providers to ensure accurate onboarding and seamless EHR integration. A valid RN/LPN license and CMS guidelines knowledge are required.

Qualifications

• RN or LPN/LVN license in good standing with active certification.
• Experience with chronic care management and telehealth.
• Knowledge of CMS CCM guidelines and HIPAA compliance.

Responsibilities

• Engage patients monthly using motivational interviewing to support health goals.
• Develop and update CCM care plans based on assessments and priorities.
• Deliver on onboarding with a white-glove approach and ensure smooth EHR setup.
• Coordinate with care teams, physicians, nurses, and social workers to align care.
• Maintain HIPAA-compliant documentation in the EHR and track engagement metrics.
