Job summary

UnitedHealth Group's UnitedHealthcare division seeks a Chronic Intake Coordinator to manage intake and coordination of chronic infusion referrals from receipt to scheduling readiness. This role serves as central contact for patients, providers, payors, and internal teams to ensure referrals progress efficiently.

The position is full-time, Monday–Friday with 8-hour shifts within 7:00am–7:00pm in respective time zones; overtime may be required during January–February as needed.

Qualifications

• High School Diploma or equivalent; must be 18+ years old.
• 2+ years in healthcare, infusion, or intake roles.
• Experience with prior authorizations and referrals.
• Strong customer service and patient communication skills.
• Proficiency with Microsoft Office and healthcare systems.

Responsibilities

• Manage chronic infusion referrals from intake to start of care readiness.
• Review referral documentation for completeness and accuracy.
• Coordinate missing information requests with providers, sales partners, and patients.
• Review payor requirements and determine authorization needs.
• Submit authorization requests and follow up until final determination.
• Collaborate with Benefits Verification, Nursing, Pharmacy, Sales to advance referrals.
• Document patient interactions and maintain HIPAA-compliant records.
• Identify barriers to care and escalate complex cases.
• Escalate issues and coordinate with providers, payors, and patients.
