Wellbox is a rapidly growing healthcare company focused on empowering people to live healthier lives through comprehensive preventative care solutions delivered by an elite team of experienced nurses. We are currently seeking tech-savvy LPN Patient Care Coordinators to conduct monthly telephonic outreach to chronically ill patients, developing personalized care plans that address their health challenges.


Essential Functions:

  1. Manage patients’ healthcare needs via virtual phone conversations.
  2. Document visits using technology platforms and EHRs.
  3. Develop personalized care plans addressing physical, mental, and preventative health.
  4. Coach patients on treatment plans, including nutrition and wellness.
  5. Connect patients with resources and prepare them for medical appointments.

Requirements:

  1. Active Compact LPN License.
  2. Minimum two years of clinical experience; care coordination experience preferred.
  3. Proficient with Electronic Medical Records and Microsoft Office.
  4. Excellent communication and problem-solving skills.
  5. Candidates who reside in Mountain Standard Time (MST) are preferred; candidates in Pacific or Central time zones will also be considered.

Position Type and Expected Hours of Work:

We are seeking full-time LPN team members who can work 40 hours per week, between the hours of 8 am – 6 pm MST, Monday – Friday.


Pay Structure:

  1. Orientation + Training (First two months): $20 hourly
  2. Post-Orientation: $22 hourly, plus bonus incentive
  3. Monthly Bonuses up to $525
  4. Referral Bonuses up to $1000
