HCA Healthcare, Inc. is seeking a Centralized Order Entry Pharmacist to work remotely in a part-time evening schedule. You will monitor, evaluate and optimize drug therapy, enter orders accurately, and ensure compliance with hospital policies. This role requires SC licensure and hospital experience is preferred.

You will screen for interactions, manage non-formulary drug handling, report adverse events and tailor care from neonatal to geriatric patients. Meditech experience is a plus.

Qualifications

• Bachelor’s degree from an accredited college of pharmacy is required.
• Pharm D is preferred.
• One (1) year of hospital experience is preferred.
• Meditech experience is preferred.
• License - State Board of Pharmacy Required.

Responsabilités

• Provide patient care activities to ensure safe and effective drug therapy.
• Accurately enter orders in the computer in a timely manner.
• Follow facility procedures for non-formulary, target, restricted and investigational drugs.
• Screen for drug interactions, allergies, order duplications, appropriate diagnosis, renal and liver function prior to order entry.
• Address facility queues in a timely manner.
• Investigate and report adverse drug events and medication incidents.
• Review and interpret physician orders using the patient profile.
• Monitor for incompatibilities, concentration and rate on IV drugs.
• Document clinical interventions and follow-up when indicated.
• Assess orders for age-specific appropriateness from neonatal through geriatric.

Formation

Bachelor’s degree in Pharmacy

Doctor of Pharmacy (PharmD) preferred
