Job summary

CareConnectMD is seeking a Clinical Documentation Integrity (CDI) Reviewer to review patient records, ensure documentation reflects the clinical condition, and support complete, compliant, and specific coding using ICD-10 and CPT guidelines. You will collaborate with providers, clinical teams, and leadership to improve documentation quality.

In this remote role, you will work with risk adjustment, quality, and operational teams to identify gaps, educate staff, and maintain HIPAA compliance

Qualifications

• RN or NP/PA with a minimum of 3 years of progressive clinical experience.
• At least two years of chart review experience for medical conditions in managed care.
• Coding certification is preferred but not required.
• Strong understanding of ICD-10-CM, CDI principles, and physician query processes.

Responsibilities

• Review high-acuity records to identify indicators, gaps, and suspect diagnoses impacting risk adjustment.
• Analyze records to ensure accurate diagnosis capture and documentation improvement.
• Communicate documentation requirements and regulatory standards to providers and stakeholders.
• Support coding validation and provider education on documentation and risk adjustment.
