Piper Companies is currently seeking a Case Manager, RN for a fully remote opportunity for a highly rated quality health insurance provider. The Case Manger, RN will be serving as an essential bridge between health care and medical coverage to advocate and increase patient's quality of care, and overall quality of life. This candidate will also need a North Carolina or compact RN license.

Responsibilities of the Case Manager, RN:

  • Work on a collaborative Health Care team to monitor utilization patterns to identify and facilitate appropiate health care delivery for selected members
  • Perform comprehensive assessments of member's health status through medical records, support needs, clinical history, and cultural needs.
  • Perform ongoing attention to member's needs including follow-up appointments, and modifications to assess progress
  • Collaborate with primary care physician, medical care coordinator, discharge services, medical director, the member, and family members to develop and coordinate plans of treatment and the proper discharge plans

Qualifications of the Case Manager, RN:

  • RN or LPN- North Carolina license required
  • Prior Telephonic Case Management experience is a plus
  • 3 + years of inpatient care experience
  • Critical care experience, including med-surg, ICU, and etc.
  • Must reside in EST time zone*

Compensation for the Case Manager, RN:

  • Annual Salary Range: $32-$37/hr based on relative experience, paid out weekly!
  • Comprehensive Benefits: Generous Benefit offerings including Medical, Dental, Vision, 401k, Sick Leave if required by law
