Job summary

Pharmacy Careers is seeking a Remote Case Management Pharmacist to coordinate care for high-risk and complex patients from home. You will conduct medication reviews, identify gaps, support prior-authorization, and educate patients and caregivers on regimens and disease management.

Collaborating with physicians, nurses, and care coordinators, you will document activities in line with health-plan standards and participate in quality initiatives to reduce hospitalizations and improve outcomes.

Qualifications

• Strong communication and collaboration skills are required.
• Experience with case management or managed care is preferred.
• Counseling and care coordination experience across clinical settings is encouraged.

Responsibilities

• Collaborate with physicians, nurses, and care coordinators to manage high-risk or complex patients.
• Conduct medication reviews to identify gaps in therapy, adherence concerns, or potential interactions.
• Support prior-authorization and appeals processes when needed.
• Educate patients and caregivers on regimens and disease-state management.
