Qualifications

• PharmD or Bachelor of Pharmacy required.
• Experience in case management or MTM preferred; hospital, ambulatory, and community pharmacists welcome.
• Strong communication and problem-solving skills essential.

Responsibilities

• Collaborate with physicians, nurses, and care coordinators to manage high-risk or complex patients.
• Conduct medication reviews to identify gaps, adherence issues, or interactions.
• Support prior-authorization and appeals processes as needed.
• Educate patients and caregivers on regimens and disease-state management.
• Document case activities and outcomes per health-plan and regulatory standards.
• Participate in quality initiatives to reduce hospitalizations and improve outcomes.
