Pharmacy Careers is seeking a Remote Case Management Pharmacist to coordinate care for high-risk patients from a home-based setting. You will review medications, support care teams, and assist with prior-authorization processes.

A PharmD or Bachelor of Pharmacy is required, with experience in MTM or managed care preferred.

Qualifications

• PharmD or Bachelor of Pharmacy required.
• Experience in case management, MTM, or managed care preferred; hospital, ambulatory and community pharmacists with strong counseling encouraged.
• Strong communication, problem-solving, and care-coordination abilities.

Responsibilities

• Collaborate with physicians, nurses, and care coordinators to manage high-risk or complex patients.
• Conduct medication reviews to identify gaps in therapy, adherence concerns, or potential interactions.
• Support prior-authorization and appeals processes when needed.
• Educate patients and caregivers on regimens and disease-state management.
• Document case activities and outcomes in compliance with health-plan and regulatory standards.
• Participate in quality initiatives aimed at reducing hospitalizations and improving outcomes.
