This role focuses on optimizing operational and administrative processes within the Pomelo clinical practice to enhance efficiency and patient experience.

Qualifications

• 2+ years healthcare administration experience, ideally having worked within virtual care.
• Familiarity with SDOH platforms and referrals.
• Results-oriented operator who moves fast.
• Meticulous attention to detail.
• Goes the extra mile for patients.
• Comfort with ambiguity.
• Ability to work cross-functionally with clinicians.
• Passionate about maternal and child health.

Responsibilities

• Provide care navigation support for patients finding providers in-network and connecting to social services.
• Monitor the member support phone line and answer patient questions.
• Complete eligibility checks and manage patient offboarding.
• Own patient scheduling including prioritization, reminders, and provider scheduling.
• Manage referrals to health plan’s Case Management team.
• Collaborate to develop new workflows to improve care coordinator efficiency.
• Other responsibilities as assigned.
• Be available to work Monday–Friday 9:00 AM to 6:00 PM ET.
