Care Manager RN

Job Description

The Care Manager evaluates the needs of members through phone calls or in-home visits to assess available resources and recommends or facilitates care plans for optimal outcomes. This may include addressing behavioral health and social determinant needs. The role involves performing outreach through various methods, including telephonic, digital, home, and site visits, to assess member needs and collaborate with resources. The Care Manager develops ongoing care plans for members with high acuity levels, identifying necessary providers, specialists, and community resources, including mental health and substance use disorders.

Responsibilities

  • Coordinate between members, family/caregivers, community resources, and care provider teams to ensure accessible services.
  • Monitor care plans, member status, and outcomes, making recommendations based on changes in treatment side effects, complications, and clinical symptoms.
  • Facilitate care coordination and collaborate with providers or specialists for timely access to needed care or services.
  • Collect, document, and maintain member information and care management activities to ensure compliance with state, federal, and third-party payer regulations.
  • Provide education to members and their families on procedures, healthcare provider instructions, treatment options, referrals, and healthcare benefits.
  • Offer feedback to leadership on opportunities to improve care and quality delivery for members cost-effectively.
  • Perform transition of care case management for Medicaid members and assist facility staff with discharge planning efforts.
  • Conduct outreaches to members within 10 business days of hospital discharge.
  • Comply with all policies and standards.

Work Environment

The position follows a Monday to Friday schedule from 8 AM to 5 PM with no overtime. The first day involves setting up equipment, meeting with HR, and preparing for a 3-4 week training period. The work environment is supportive, encouraging collaboration and compliance with policies and standards.

Pay and Benefits

The pay range for this position is $35.00 - $40.00/hr.
