Job summary

Cambia Health is seeking a Remote Care Management Nurse to support members from home across eligible western states. The role focuses on case management, disease management and care coordination to deliver person-centered, cost-efficient care.

You will collaborate with physicians, care teams, and members to plan and monitor care within licensure scope. Ideal candidates will have an RN license, 3+ years of clinical or care-management experience, and eligibility for licensure in Idaho, Oregon,

Qualifications

• RN license (active, unrestricted) in US state or territory
• 3+ years direct clinical care or case/disease management
• Eligible for licensure in ID, OR, UT and WA

Responsibilities

• Conducts case management activities including assessment, planning, implementation, coordination, monitoring and evaluation.
• Applies clinical judgment to ensure compliance with medical policy and guidelines.
• Collaborates with physicians, internal/external customers and other departments to resolve issues and drive quality improvement.
• Responds to inquiries professionally while protecting confidentiality of sensitive information.
• Documents accurately and timely to meet performance standards and timelines.
• Coordinates resources and prioritizes assignments to meet goals and timelines.
