Chronic Care Staffing (CCS) is seeking a Remote Care Coordinator (CMA) to join our growing healthcare team. As a Care Coordinator, you'll play a vital role in helping patients manage their chronic conditions through consistent communication, education, and coordination with their healthcare providers.

You'll be responsible for making monthly calls to patients, assessing their health status, promoting self-management, and serving as a trusted point of contact between patients, caregivers, and medical professionals — all from the comfort of your HIPAA-compliant home office.

About The Role

The base pay range for this position is $35,360.00/yr - $41,600.00/yr. Your actual pay will be based on your skills and experience.

Key Responsibilities
  • Conduct monthly Chronic Care Management (CCM) calls or verbal enrollments.
  • Perform services such as Health Risk Assessments, Transitional Care Management, and Remote Patient Monitoring.
  • Communicate effectively with patients and family members about chronic conditions, medications, barriers to care, and treatment plans.
  • Collaborate with providers, staff, and other healthcare professionals to ensure coordinated care.
  • Promote adherence to personalized care plans and patient health goals.
  • Support patients with medication reconciliation and education.
  • Connect patients to community resources to enhance well-being and satisfaction.
  • Maintain compliance with CMS guidelines and organizational regulations.
  • Uphold CCS's reputation for professional, compassionate, and patient-focused care.
  • Meet expected call volume and maintain thorough documentation in EHR systems.
Requirements
Required Qualifications
  • Unencumbered active CMA or RMA certificate
  • Active BLS certification
  • High proficiency in EHR systems
  • Recent experience in a clinical or CCM setting
  • Proficiency in Google Suite
  • Ability to plan, organize, and work independently in a HIPAA-compliant home office (locked door, distraction-free, two monitors, high-speed internet)
  • Excellent verbal and written communication skills
  • Strong clinical reasoning, problem-solving, and critical thinking abilities
Preferred (Nice-to-Have) Qualifications
  • Experience in care coordination, chronic care management, transitional care, or remote patient monitoring
  • Familiarity with community resource navigation to support patient care
  • Experience providing patient/family education on chronic conditions and medications
  • Background in quality measures, reporting, or CMS compliance
  • Demonstrated team-player mindset in a remote environment
  • Additional coursework or certification related to care or case management
