Qualifications

• Minimum 1-2 years experience in telehealth, remote patient monitoring, or chronic care management.
• ACSM-CEP preferred for Exercise Physiologists; willing to sit for the CEP within 6 months.
• 1-5 years of prior experience in cardiac education and care management required.

Responsibilities

• Guide participants in 1:1 video calls and group education sessions.
• Develop comprehensive treatment plans tailored to individual needs.
• Engage patients weekly to provide coaching and modify treatment plans.
