Job summary

<p>Aviary is seeking a Clinical Chart Abstractor to work fully remote. This part-time contractor role is responsible for reviewing patient EMR records for medical eligibility in cardiac recovery. You will collaborate with the enrollment team, ensuring accurate communication of patient eligibility.</p><p>The ideal candidate has extensive clinical experience, proficiency in EMR systems, and active RN or LPN licensure. Schedule is flexible with 15-20 hours per week, primarily in the afternoon and evening.</p>

Qualifications

• 3-5 years of clinical experience in cardiac care or chronic disease management.
• 3 years of EPIC or general EMR experience.
• 1 year of medical terminology knowledge.

Responsibilities

• Review patient charts for medical eligibility/ineligibility.
• Identify and create cardiac rehab referral orders as needed.
• Communicate patient eligibility to the enrollment team.
