Qualifications

• Minimum 8+ years of clinical practice.
• Minimum 5+ years as a physician executive with managed care experience.
• MD or DO with current CA medical license.
• Board certification in a clinical specialty.
• Willingness to travel within California.

Responsibilities

• Provide clinical leadership and physician oversight for the CA transition-support program.
• Establish clinical standards and escalation pathways.
• Advise on continuity of care and post-discharge needs.
• Collaborate with nurses, social workers, care coordinators, and staff.
• Develop clinical protocols and training materials.
