Liaisons Community Care is seeking a detail-oriented Note Reviewer to review behavioral health documentation for accuracy, completeness, and compliance. This remote role collaborates with clinical and administrative staff to identify documentation gaps and help improve client records.

Responsibilities include reviewing notes, treatment plans, and discharge summaries, verifying billing data and signatures, and providing constructive feedback to staff to ensure high-quality records.

Formación

• Bachelor’s degree in healthcare field required; master’s preferred.
• Experience reviewing healthcare documentation required; behavioral health experience preferred.
• Knowledge of behavioral health documentation standards, DSM-5 terminology, CPT codes, and billing documentation requirements.
• Strong attention to detail, sound judgment, and clear written communication.
• Ability to manage priorities and work independently in a remote setting.
• Experience using electronic health record systems.
• Ability to meet applicable background screening requirements.

Responsabilidades

• Review progress notes, treatment plans, assessments, and discharge summaries for information and clarity.
• Ensure documentation reflects services provided, treatments, and progress toward goals.
• Verify signatures, dates, client IDs, and billing information.
• Identify missing information or discrepancies and provide constructive feedback.
• Track review findings and follow up on needed corrections.
• Support documentation audits and quality improvement efforts.
• Protect confidential client information and HIPAA compliance.
