Job summary

Mindoula Health Inc in Oregon is expanding its Collaborative Care Program to Corvallis. We seek an Associate Behavioral Health Integration Clinician to join a multidisciplinary team providing short-term, evidence-based care under supervision.

You will screen for mental health and substance use, support medication adherence, provide patient education, participate in weekly psychiatric case consultations, and document in the EHR.

Qualifications

• Master’s degree in Social Work, Counseling, Psychology, or related field.
• Associate level licenses: CSWA (Clinical Social Work Associate).
• 3+ years of experience working with adult populations with chronic medical + behavioral needs.
• Experience with EMRs and telehealth workflows; strong technology proficiency.
• Located in Oregon (majority of services are remote).

Responsibilities

• Screen/assess for mental health and substance use disorders.
• Provide short-term, evidence-based interventions (BA, PST, MI, etc.).
• Support medication adherence and monitor side effects.
• Provide education and self-management support.
• Track clinical response via in-person, video, or phone and adjust plan.
• Refer for specialty care when appropriate.
• Participate in weekly psychiatric case consultation.
• Document in EHR consistent with model workflows.
• Re-engage patients who fall out of care.
• Practice under supervision of independently licensed clinicians per Oregon regulations.
