Shields Health Solutions is hiring an Ambulatory Clinical Pharmacy Specialist to elevate patient care through virtual assessments and evidence-based treatment plans under a CPA. This remote role supports the Centralia/Olympia/Lacey area with potential locations at Providence Centralia and St. Peter hospitals.

Qualifications

• Doctor of Pharmacy (PharmD) required and WA pharmacist license.
• Residency training options include PGY2 Ambulatory Care or PGY1 with CPA and ambulatory practice.
• 3+ years of ambulatory practice with disease state management preferred.

Responsibilities

• Provide comprehensive medication management and reconciliation.
• Collaborate with physicians and care teams under a CPA to plan and monitor therapy.
• Monitor therapy effectiveness and safety; support complex prior authorizations.
