As part of a multidisciplinary team, you will deliver wraparound care, collaborate with specialists, and support individuals to improve functioning and community participation. Flexible scheduling and per diem opportunities are available.

Qualifications

• RN with clinical and community based experience working with individuals with mental health conditions.
• Experience in community-based mental health services preferred.
• Relief/Per Diem availability and schedule flexibility.

Responsibilities

• Provide wraparound, mobile community-based care to individuals with serious mental health conditions.
• Collaborate with a multidisciplinary team to coordinate and integrate services.
