Open Sky Community Services seeks Relief Counselors to provide direct support aimed at helping individuals facing mental health challenges live independently and reach beyond perceived limits.

Responsibilities include implementing person-centered plans, facilitating community access, promoting growth and relationships, and completing required documentation in a supportive, trauma-informed environment.

Qualifications

• High School Diploma or GED required.
• Valid Driver’s License and acceptable driving record required.
• Reliable with a willingness to learn and adapt.

Responsabilités

• Assist in developing and implementing individualized, person-centered support plans.
• Assist individuals in accessing community resources and/or provide transportation as needed.
• Implement activities that promote skill development, community engagement, personal growth, and meaningful relationships.
• Review and complete all required documentation.
