Open Sky Community Services is seeking Relief Counselors to support individuals facing mental health challenges, helping them live independently and participate in community life. You’ll assist with care plans, transportation, and daily living activities while documenting progress in a trauma-informed, inclusive environment.

Qualifications

• High School Diploma, GED or equivalent, required.
• Valid Driver’s License and acceptable driving record, required.
• Reliability, willingness to learn, and being open to new opportunities.

Responsibilities

• Assist in developing and implementing individualized, person-centered support plans.
• Assist individuals in accessing community resources and/or provide transportation as needed.
• Implement activities that promote skill development, community engagement, personal growth, and meaningful relationships.
• Review and complete all required documentation.
