Job summary

Open Sky Community Services seeks a Relief Counselor to support individuals with mental health challenges, helping them live independently and engage in meaningful activities.

You will provide direct care, assist with daily living tasks, and accompany clients to community resources, with an emphasis on safety, dignity, and person-centered planning. This hourly role offers flexibility to fit your schedule, including evenings and weekends.

Qualifications

• High School Diploma, GED or equivalent required.
• Valid Driver’s License with acceptable driving record required.
• Reliability and willingness to learn; openness to new opportunities.

Responsibilities

• Assist in developing and implementing individualized, person-centered support plans.
• Assist individuals in accessing community resources and/or provide transportation as needed.
• Implement activities that promote skill development, community engagement, personal growth, and meaningful relationships.
• Review and complete all required documentation.

Other Key Responsibilities

• Assist in developing and implementing individualized, person-centered support plans.

• Review and complete all required documentation.

The organization recognizes skills, location, training, and populations served in certain programs by offering an additional hourly differential. Ask your recruiter if this position qualifies for an additional hourly differential!
