Qualifications

  • Ability to prioritize and manage multiple tasks simultaneously, and to effectively anticipate and respond to issues as needed in a dynamic work environment.
  • Knowledge of Microsoft Word, Excel, and Outlook.

Responsibilities

  • Monitor timeliness of outstanding requests; process requests per Federal/State statute and Hospital policy.
  • Validate requester authority for medical record information.
  • Retrieve and prepare medical record documentation; coordinate with sites and vendors.
  • Communicate with customers; provide guidance on procedures and consent requirements.
  • Track and log requests per procedures; analyze patterns and variances.
  • Uphold Hospital Customer Service standards; perform other duties as assigned.

Job description

Purpose of Position

To receive and process requests for medical record information in accordance with Federal and State Statute and Hospital Policy, while protecting patient confidentiality and right to access.

Responsibilities
  • Monitor and control the timeliness of outstanding requests for medical information. Receive and process requests for medical record information in accordance with Federal and State statute and Hospital policy, ensuring that all documentation is obtained and submitted in accordance with departmental procedures.
  • Validate that requestor has the authority to obtain medical record information in accordance with Federal and State Statute, third party contracts and/or Hospital Policy.
  • Reviews, retrieves and prepares medical record documentation in response to requests; contacts staff at other CCHC sites who maintain decentralized medical record files as well as off‑site storage vendors to obtain medical record information.
  • Communicates and interacts with customers who request medical record documentation, ensuring requests are responded to appropriately and in a timely manner. As needed, provides instructions to requestors related to procedures for obtaining medical record information and patient consent requirements.
  • Tracks and logs requests for medical record documentation in accordance with departmental procedures, trending and interpreting patterns and variances.
  • Performs all duties and interacts with others in accordance with the Hospital's Customer Service standards.
  • Performs other work related duties as assigned.
