CJR is seeking a Reintegration Mentor in Bridgeport, CT to support adolescents transitioning from treatment. The role involves guiding treatment plans, coordinating appointments, and offering aftercare for up to 12 months post-discharge.

The position requires engaging with youth in both the facility and community, working with families and staff, and maintaining connections for successful reintegration. Flexibility in hours, including evenings and weekends, and the ability to work independently

Qualifications

• High school diploma or equivalent.
• Lived/ firsthand experience related to this position, and experience, training, and a history of working with adolescents - Required
• Understanding unique issues and needs of adolescents - Required
• Flexible hours/weekends and evenings - Required
• Valid driver’s license to utilize a company vehicle - Required
• Skill in Advocacy, Crisis Intervention and ability to function independently
• Ability to speak Spanish a plus

Responsibilities

About This Job:

The Reintegration Mentor provides ongoing support and guidance to adolescents transitioning from treatment, serving as a role model and advocate. The role involves facilitating treatment plans, coordinating appointments and community activities, and offering aftercare for up to 12 months post-discharge. The position requires engaging with youth both in the facility and in the community, working collaboratively with staff and families, and maintaining connections to promote successful reintegration. Flexibility in hours, including evenings and weekends, and the ability to work independently are essential. Overall, the position aims to support adolescents’ ongoing development and stability as they transition back into the community.

Responsibilities and Duties:

The Reintegration Mentor will be a role model and will reinforce the adolescent’s individual treatment plan as well as continued support for the adolescent and family after completion of the program. Transport the adolescent to appointments or recreation activities to prepare them for discharge. Monitor home visits in the community. Work with facility staff to meet with youth one on one as well as facilitate the ARISE curriculum. Provide aftercare to youth for 12 months post discharge.

• Engage with youth in the facility
• Maintain relationships with facility staff on behalf of the adolescents
• Facilitate ARISE Curriculum
• Advocate for youth and their families
• Attend court or providers meetings
• Play a significant role in the client’s treatment team
• Work on a flexible schedule, which includes weekend and evening hours
• Share On-Call hours with other staff
• Develop knowledge base of community resources across Connecticut
• Maintain connection with youth and family throughout treatment and beyond
• Perform aftercare for RESTS clients for one year

Requirements and Qualifications:

Schedule and Rate of Pay:

• Salary pay range is $60,000-$65,000
  • Actual pay is determined based on a variety of job-related factors, including skills, education, training, credentials, experience, certifications, scope, and performance, as well as market rates, internal equity, budget constraints, the role’s responsibilities, candidate’s unique qualifications, potential for growth, benefits, and organizational policies.
• Schedule will include 35 hours per week
  • Some evenings may be required based on family’s schedules
• Schedule flexibility will be required to ensure the youth and family’s needs are met
