Job summary

Maple Grove Senior Living is seeking an experienced Rehabilitation Services Manager to lead our therapy program and support exceptional outcomes for residents receiving skilled rehabilitation and long-term care services.

This role provides leadership over physical, occupational, and speech therapy teams, ensuring high-quality resident care, regulatory compliance, and strong interdisciplinary collaboration within a continuum-of-care community.

Qualifications

• Degree from an accredited Physical Therapy, Occupational Therapy, or Speech-Language Pathology program with active Kentucky licensure in good standing.
• Previous rehabilitation leadership or management experience preferred.
• Experience in skilled nursing, long-term care, or post-acute rehab preferred.

Responsibilities

• Lead PT, OT, and Speech Therapy services.
• Oversee daily rehab operations, staffing, scheduling, and workflow.
• Promote resident-centered rehabilitation and positive outcomes.
• Monitor therapy performance, documentation, productivity, and compliance.
• Collaborate with nursing, physicians, administration, and the interdisciplinary team.
• Recruit, onboard, develop, and retain rehab team members.
• Monitor department financial performance and manage resources.
• Identify opportunities to strengthen rehab services and census.
• Maintain compliance with federal, state, and facility requirements.

Job Overview

Maple Grove Senior Living is seeking an experienced Rehabilitation Services Manager to lead our therapy program and support exceptional outcomes for residents receiving skilled rehabilitation and long-term care services.

The Rehabilitation Services Manager provides leadership and oversight to our physical, occupational, and speech therapy teams while ensuring high-quality resident care, regulatory compliance, strong interdisciplinary collaboration, and effective department operations.

Requirements

Requirements

• Degree from an accredited Physical Therapy, Occupational Therapy, or Speech-Language Pathology program preferred, with applicable active Kentucky licensure in good standing.
• Previous rehabilitation leadership or management experience strongly preferred.
• Skilled nursing, long-term care, or post-acute rehabilitation experience preferred.
• Strong understanding of rehabilitation operations, documentation, reimbursement, and regulatory requirements.
• Demonstrated leadership, communication, financial management, and problem-solving skills.
• Ability to develop strong relationships with residents, families, staff, and healthcare partners.
• Ability to meet the physical requirements of the position, including prolonged standing/walking and assisting with lifting or transferring up to 50 pounds as needed.
