Job summary

Maple Grove Senior Living in Shelbyville, KY is seeking an experienced Rehabilitation Services Manager to lead our therapy program and support exceptional outcomes for residents receiving skilled rehabilitation and long-term care services.

You will oversee Physical Therapy, Occupational Therapy, and Speech Therapy teams, manage daily operations, staffing, scheduling, and documentation while ensuring regulatory compliance and strong interdisciplinary collaboration.

Qualifications

• Active Kentucky licensure in good standing is required.
• Leadership or management experience in rehabilitation preferred.
• Experience in skilled nursing, long-term care, or post-acute rehabilitation.
• Strong understanding of rehabilitation operations, documentation, reimbursement, and regulatory requirements.
• Ability to develop strong relationships with residents, families, staff, and healthcare partners.

• Lead and supervise Physical Therapy, Occupational Therapy, and Speech Therapy services.
• Oversee daily rehabilitation operations, staffing, scheduling, and workflow.
• Promote resident-centered rehabilitation and positive clinical outcomes.
• Monitor therapy performance, documentation, productivity, and regulatory compliance.
• Collaborate with nursing, physicians, administration, and the interdisciplinary team to coordinate resident care.
• Recruit, onboard, and develop rehabilitation team members.
• Monitor department financial performance and manage resources effectively.
• Identify opportunities to strengthen rehabilitation services and program performance.

Maple Grove Senior Living is seeking an experienced Rehabilitation Services Manager to lead our therapy program and support exceptional outcomes for residents receiving skilled rehabilitation and long-term care services.

Requirements

Requirements

• Degree from an accredited Physical Therapy, Occupational Therapy, or Speech-Language Pathology program preferred, with applicable active Kentucky licensure in good standing.
• Previous rehabilitation leadership or management experience strongly preferred.
• Skilled nursing, long-term care, or post-acute rehabilitation experience preferred.
• Strong understanding of rehabilitation operations, documentation, reimbursement, and regulatory requirements.
• Demonstrated leadership, communication, financial management, and problem-solving skills.
• Ability to develop strong relationships with residents, families, staff, and healthcare partners.
• Ability to meet the physical requirements of the position, including prolonged standing/walking and assisting with lifting or transferring up to 50 pounds as needed.
