Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Administer medications and monitor patient response.
• Oversee wound care and IV care, ensuring safe, effective treatment.
• Promote mobility, safety, and independence through patient-centered interventions.
• Collaborate with an interdisciplinary team, including therapists and physicians.
• Participate in continuous learning, quality improvement, and adoption of advanced rehab practices.
• Mentor support staff and educate patients and families on care and recovery.
