Job summary

UC Davis Rehabilitation Hospital, part of Lifepoint Health, is seeking a Licensed Practical Nurse (LPN) to join the rehabilitation team in Sacramento. You will administer medications, assist RNs with assessments, and contribute to interdisciplinary care aimed at restoring independence after illness or injury.

Ideal candidates have an accredited LPN program, current LPN licensure, CPR/BLS, and at least 1 year of medical/surgical experience in acute care.

Qualifications

• Graduate of an accredited Licensed Practical/Vocational Nurse program.
• Current state licensure as an LPN/LVN.
• CPR/BLS certification required; IV Certification highly preferred or must be obtained within 6 months.
• Minimum one year of Medical/Surgical experience in an acute care setting preferred.

Responsibilities

• Administer medications safely and accurately based on patient age and condition.
• Assist RN with initial assessments and patient reassessments per policy.
• Identify and implement rehabilitation nursing interventions.
• Perform timely Quality Improvement assessments under RN supervision.
• Deliver care based on patient response and update the interdisciplinary plan accordingly.
• Assess and reassess pain; educate patients and families on pain management.
• Develop and implement individualized teaching plans based on patient and family learning needs.
• Perform other duties as assigned.
