The role includes scheduling care per Plan of Care, coordinating staffing with the Division Rehab coordinator, ensuring accurate documentation, and collaborating with the interdisciplinary team to uphold outstanding patient care and safety.

Qualifications

• Associate’s degree in Physical Therapy or Occupational Therapy is required.
• Current, unencumbered Georgia PTA/OTA license.
• Prefer two years of skilled nursing facility rehab experience.

Responsibilities

• Oversees daily rehab department operations and provides clinical case management.
• Provides direct patient care and support per Plan Of Care.
• Schedules patient care according to Plan Of Care established by Therapists.
• Collaborates with Rehab coordinator to schedule appropriate rehab staffing.
