Job summary

Evolve Therapy Services is seeking a Director of Rehabilitation for an in-house Skilled Nursing Facility in Clarion, PA. You will oversee therapy operations, ensure regulatory compliance, and lead an interdisciplinary team to deliver high-quality patient care.

Responsibilities include program oversight, staff training, adherence to PDPM/MDS processes, and ensuring proper documentation and billing. Strong leadership and communication skills are essential for success in this role.

Qualifications

• Degree in Physical Therapy, Occupational Therapy, or Speech Language Pathology.
• Active license in state(s) of practice (PT/PTA/OT/OTA/SLP).
• Ongoing continuing education to maintain licensure.
• Experience with PDPM and MDS management preferred.
• Excellent leadership and communication skills.
• Results-oriented with strong critical thinking and problem-solving skills.

Responsibilities

• Oversee and manage all aspects of the therapy program in the facility/facilities.
• Ensure adherence to regulatory requirements, company policies, procedures and guidelines.
• Establish facility-specific procedures and train therapy staff.
• Ensure therapy services meet regulatory and company standards and productivity targets.
• Ensure documentation and billing comply with regulatory requirements.
• Communicate professionally with patients and families and address requests.
• Perform other duties as assigned to support organizational goals.
