Job summary

Apploi seeks a Director of Rehabilitation to lead our therapy team and drive clinical excellence for residents in a full-time role. You will supervise therapy services, ensure regulatory compliance, and partner with nursing and administration to deliver high-quality care.

You will oversee documentation, outcomes, and program development while mentoring therapists and maintaining compliance with state and federal guidelines in a long-term care setting.

Qualifications

• The Rehab Director must possess a valid, unrestricted license in his/her therapy discipline in the state.
• Educational requirements of licensure.
• Valid driver’s license in the state of employment.
• At least three (3) years of experience in a supervisory role in a hospital, nursing home, or related health care facility.
• Knowledgeable of skilled nursing home regulations, laws, and guidelines for long-term care.

Responsibilities

• Plans, develops, organizes, implements, evaluates and directs the rehabilitation department in accordance with laws and practice acts.
• Directs the execution of rehabilitation services per physician orders and professional associations.
• Monitors key rehabilitation indicators and maintains Quality Assurance and Performance Improvement processes.
• Evaluates outcomes with therapists and leaders and initiates actions as needed.
• Leads processes to ensure timely screening, evaluation, treatment, discharge and documentation for residents.
