Riverside Community Care is seeking a Rehabilitation Counselor for our PACT program in Needham, MA. The role focuses on community-based rehabilitation, clinical treatment, employment support, and housing and wellness services tailored to adults with severe mental illness.

Join a multidisciplinary team delivering 24/7 support across a network of community services. The position emphasizes client-centered care, resiliency, and recovery in collaboration with the Department of Mental Health and MBHP

Qualifications

• Bachelor’s Degree and one year of related experience required.
• Experience working with adults with severe mental illness is preferred.
• Criminal Justice or Vocational Rehabilitation background is a plus.

Responsibilities

• Provide guidance, education, supervision, and direct supports as part of a multidisciplinary team.
• Assist individuals served to maintain health and safety and develop goal-related skills.
• Coordinate services with community partners and family/caregivers as needed.
• Support transportation to participate in community-based activities.

Riverside Community Care is seeking a Rehabilitation Support Counselor member to join our community-based Program for Assertive Community Treatment (PACT) program based in Needham and serving the surrounding area.

Pact provides individuals referred by the Department of Mental Health or MBHP with access to community based rehabilitation, clinical treatment, employment, housing, wellness and peer services, with ancillary supports that will be coordinated in partnership with the individual. Services are designed to be flexible based on the needs of the persons served, and to promote resiliency and recovery for all participants. PACT is a 24/7 program and services are provided 365 days per year.

Working as part of a multi-disciplinary team, staff will provide guidance, education, supervision, and direct supports to allow individuals served to maintain their physical and emotional health, live safely in the community, and develop the skills necessary to meet their goals. Interactions will be guided by an appreciation for those being served as unique individuals and adults, with the expectation and goal of mutual learning and growth. Services will be provided in keeping with the Division Mission Statement and PACT Guidelines and are consistent with Riverside and Department of Mental Health standards and requirements.

Pay Rate: $19.56/hourly

Required Skills
