University Health in Boerne, TX is seeking a Nurse Case Manager to coordinate with the interdisciplinary treatment team, evaluate admissions to the inpatient rehabilitation unit, and ensure timely delivery of services.
The role includes Pre-Admission Screening, tracking outcomes, advocating for patients across the care continuum, and developing comprehensive therapy programs for medically referred patients.
Responsible for coordinating with the interdisciplinary treatment team to evaluate and screen patients for admissions to the inpatient rehabilitation unit and Rehabilitation Service. Conducts Pre-Admission Screening and facilitates the delivery of services, evaluates effectiveness, tracks outcomes and functions as the patient advocate to identify and communicate health care needs across the healthcare continuum. Develops, implements, and evaluates patients for comprehensive therapy program for all medically referred patients. Assists with educational, administrative, managerial, and quality control duties as assigned.
BSN preferred. A minimum of 2 years nursing experience preferred. Work experience in rehabilitation case management, utilization review or hospital quality assurance is preferred. Bilingual in Spanish/English is preferred.
Current registration with the Texas State Board of Nurse Examiners is required. Case Management Certification (CCM or ANCC) is preferred. Current American Heart Association, Basic Cardiac Life Support and Health Care Provider card required.
