Responsibilities
  • Safety: Monitors and ensures safety 24/7 through various checks; supports crisis prevention, intervention, and resolution.
  • Relationship Building: Builds trust and empathy with clients, fostering partnerships with clients, community, and professionals.
  • Activities and Daily Living: Helps clients develop problem-solving, communication, and independence skills; supports social interests and Activities of Daily Living; assists with bowel and bladder programs, feeding, wound care, and transfers.
  • Health: Monitors vitals, promotes positive behavior, attends medical appointments, and implements care plans with proper documentation.
  • Community Integration: Connects clients with community opportunities, supporting employment, education, and volunteer pursuits.
