Job summary

Community Counseling Center of Mercer County is seeking a dedicated professional to support the Community Support Services team, providing coverage for OVR, CEERS and PETS programs. The role includes job development, coaching, casework and direct service to a caseload of 9-15 clients, with data entry in Credible and CWDS and regular collaboration with mental health professionals and families.

Qualifications

  • Bachelor's degree in social services field or combination with a Bachelor's Degree and minimum of 2 years human services experience.
  • Act 33 and 34 Clearances required.
  • Must maintain a current driver's license and vehicle insurance as will be required to transport clients.

Responsibilities

  • Obtain ACRE Basic Employment Services Certificate within 6 months of hire.
  • Network with mental health professionals, consumers, employers and family members to provide effective services.
  • Perform job development in the community to create paid employment opportunities and work-based learning opportunities and conduct groups.
  • Provide pre-vocational activities and job seeking skills training per client need to aid them in identifying strengths, limitations and suitable vocational goals.
  • Provide job coaching for all persons who find employment and need on-site and off-site assistance, including outside normal hours and weekends.
  • Collect indirect and direct observation data at the job site per client need.
  • Provide at least 80% of direct service per week.
  • Complete employer satisfaction surveys after 30 and 90 days.
  • Provide on- and off-site support for consumers including referrals and linkages after fading.
  • Utilize CWDS and Credible for documentation and billing for all OVR activities.
  • Develop creative ways to engage consumers to stay involved and improve work skills and social skills.
  • Function as an effective team member in developing Job Development Plans for consumers.
  • Serve as a role model for professional conduct and grooming; participate in on-site and community activities.
  • Complete and submit all paperwork for OVR weekly or monthly; document in Credible and CWDS; set goals for consumers.
  • Attend weekly staff meetings with supervisor.
  • Meet with participants in both programs individually on a regular basis.
  • Provide casework services to a caseload of 9-15 clients.
  • Schedule appointments and coordinate follow-up care.
  • Input all necessary information in Credible and CWDS; submit reports timely.
  • Adhere to weekly schedule or notify supervisor of changes.
  • Maintain all appointments or notify consumer of delays.
  • Attend relevant team meetings and trainings.
  • Be knowledgeable about safety procedures on and off site.
  • Maintain certifications and trainings via Relias.
  • Work with Drop-In Center staff in weekly meetings.
  • Monitor and evaluate job performance and assist in transitions to competitive employment.
  • Schedule and conduct work support meetings.
  • Provide all Pre-ETS services to qualified youth and young adults.
  • May be cross-trained in other vocational areas as needed.
  • Job coaching duties include lifting up to 25 pounds, standing, sitting, bending, reaching and stairs; may work outdoors.
  • Be flexible in service provision (in person, via Zoom, over the phone, or in the community) to meet client needs and health/safety requirements.

QUALIFICATIONS

This position functions as part of the Community Support Services team providing staff coverage for both OVR, CEERS and PETS programs. A Bachelor's Degree in Social Services field or a combination of a Bachelor's Degree and minimum of at least 2 years human services experience. Must have good interpersonal and organizational skills in order to communicate effectively with referral and funding sources, family members, employers and clients. Act 33 and 34 Clearances required. Must maintain a current driver's license and vehicle insurance as will be required to transport clients.

RESPONSIBILITIES
  1. Must obtain ACRE (Association of Community Rehabilitation Educators) Basic Employment Services Certificate within 6 months of hire.
  2. Must be able to network and maintain positive working relationships with mental health professionals, consumers, employers and family members in order to provide effective services.
  3. Will perform job development in the community as needed in order to create paid employment opportunities, and work- based learning opportunities and conduct groups.
  4. Will provide pre-vocational activities and job seeking skills training per client need to aid them in identifying strengths, limitations and suitable vocational goals.
  5. Will provide job coaching for all persons who find employment and need on site (and off site) assistance regardless of the schedule, ie.outside of the normal work hours and on weekends.
  6. Collect both indirect employer observation and direct observation data at the job site, per client need.

  9. Will provide on and off site support for consumers to include making all necessary referrals and linkages to insure adequate support after fading.
  10. Utilize C.W.D.S. and Credible in order to provide documentation and billing for all OVR activities.
  11. Develop creative ways to challenge consumers to stay involved, increase work skills, improve work habits and socialize in a way appropriate to the community
  12. Function as an effective team member in the development of Job Development Plans for the consumers.
  13. Serves as an effective role model for good work habits, good grooming and appropriate social skills. Participates in activities on site and in the community in a manner that is professional.
  14. Completes and submits all paperwork either weekly or by the last day of each month for OVR. All other documentation will be done in the Credible and CWDS System. Completes initial assessments and all reviews, as needed, to set goals for the consumers to maintain their life in the community.
  15. Will attend weekly staff meeting with supervisor.
  16. Meets with participants in both programs individually on a consistent and regular basis.
  17. Provides casework services to a caseload of 9-15 clients.
  18. Schedules appointments and coordinates follow-up care to meet the needs of the clients.
  19. Inputs all necessary information in the Credible and CWDS System to maintain a client in services. Submits reports for in a timely fashion according to date ranges
  20. Will adhere to the schedule submitted at the beginning of each week, or will notify supervisor of changes.
  21. Will keep all appointments scheduled or will notify consumer of delay.
  22. Attends relevant team meetings and training workshops, as assigned, to advance skills.
  23. Is knowledgeable about safety procedures on and off site.
  24. Maintains relevant and required certifications and trainings via Relias.
  25. Works with employees at the Drop- In Center by attending weekly staff meetings and discussing relevant work issues.
  26. Monitors and evaluates job performance by caf and cleaning employees, assists in transitioning f when appropriate to competitive employment and documents units of services.
  27. Schedules and conducts work support meetings.
  28. Provides all services under thePre-ETSservice line to qualified youth and young adults.
  29. This position may be cross trained in other vocational area programs as needs occur.
  30. This position requires job coaching consumers and essential functions of this position require lifting up to 25 pounds, standing, sitting, bending, reaching and ascending or descending stairs. May need to work in outdoor weather conditions.
  31. Must be flexible in provision of service , for example; in person, via zoom, over the phone or in the community to suit client needs along with the health and safety due to unforeseen circumstances.
