Job summary

Eagle Crest Communities is seeking a Rehabilitation Registered Nurse (Rehab RN) to join our on-site Bethany Home team in Wisconsin. This role focuses on assessments, care planning, and collaboration with therapists to restore function and independence for residents.

Qualifications include RN license, BLS, and 2 years in rehab; BSN and CRRN preferred. The position offers full-time hours, day shift, with comprehensive benefits and supportive, multidisciplinary collaboration.

Qualifications

• Valid and current Registered Nurse (RN) license in the state of practice.
• BSN degree preferred and beneficial.
• Minimum 2 years of nursing experience in rehabilitation or related setting.
• CRRN certification preferred or willingness to obtain.
• BLS certification required.
• ACLS certification preferred.

Responsibilities

• Conduct comprehensive patient assessments to determine rehabilitation needs.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Administer medications and treatments as prescribed by physicians.
• Monitor patient progress and adjust care plans to achieve rehab goals.
• Educate patients and families about treatment plans and self-care techniques.
• Collaborate with therapists and rehab professionals to optimize outcomes.
• Document patient care activities and maintain accurate medical records.
• Provide emotional support to patients and families.
• Participate in multidisciplinary team meetings to coordinate care.
• Ensure compliance with health and safety regulations and protocols.
• Assist in discharge planning to ensure continuity of care.

Description

In this vital role, you will collaborate with interdisciplinary teams to provide personalized care, support recovery, and improve quality of life for individuals in a nurturing, dynamic environment.

Responsibilities

• Conduct comprehensive patient assessments to determine rehabilitation needs.
• Develop and implement individualized care plans in collaboration with the healthcare team.

Qualifications

• Valid and current Registered Nurse (RN) license in the state of practice
• Bachelor of Science in Nursing (BSN) preferred
• Minimum of 2 years of nursing experience in a rehabilitation or related setting
• Certification in Rehabilitation Nursing (CRRN) preferred or willingness to obtain
• Basic Life Support (BLS) certification required
• Advanced Cardiovascular Life Support (ACLS) certification preferred
• Strong knowledge of rehabilitation nursing principles and practices
• Proficiency in electronic medical records (EMR) systems
• Excellent communication and interpersonal skillsAbility to work independently and as part of a multidisciplinary team
• Strong problem-solving and critical thinking skills
• Ability to lift and move patients, and perform physically demanding tasks
• Availability to work flexible shifts, including nights, weekends, and holidays as needed
