Bethany Home in Wisconsin is seeking a Rehab Registered Nurse (Rehab RN) for a full-time, on-site role. You will assess rehab needs, develop care plans, administer treatments, and collaborate with a multidisciplinary team to support patient recovery and independence.

Ideal candidates have an active RN license in Wisconsin, a BSN preferred, and at least 2 years in rehab settings. Flexible shifts and patient-centered care are essential to success.

Qualifications

• Active RN license in Wisconsin.
• BSN preferred.
• Minimum of 2 years nursing in rehab or related setting.
• CRRN preferred or willingness to obtain.
• BLS certification required.
• ACLS preferred.
• Experience with EMR systems.
• Strong communication and interpersonal skills.
• Ability to lift/move patients and perform physically demanding tasks.
• Flexible shifts including nights, weekends, and holidays.

Responsibilities

• Conduct comprehensive patient assessments to determine rehab needs.
• Develop and implement individualized care plans with the team.
• Administer medications and treatments as prescribed.
• Monitor progress and adjust plans to meet rehab goals.
• Educate patients and families on treatment plans and self-care.
• Collaborate with PT/OT and rehab staff to optimize outcomes.
• Document care activities and maintain medical records.
• Provide emotional support to patients and families.
• Participate in multidisciplinary team meetings to coordinate care.
• Ensure compliance with health and safety regulations.
• Assist in discharge planning for continuity of care.

Hours: 6:00 A.M. to 2:30 P.M

Brief Summary of Purpose

Join our compassionate healthcare team as a Rehabilitation Registered Nurse (Rehab RN), where your expertise and dedication will be valued in helping patients achieve their highest level of function and independence. In this vital role, you will collaborate with interdisciplinary teams to provide personalized care, support recovery, and improve quality of life for individuals in a nurturing, dynamic environment.

Responsibilities

• Conduct comprehensive patient assessments to determine rehabilitation needs.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Administer medications and treatments as prescribed by physicians.
• Monitor patient progress and adjust care plans accordingly to achieve rehabilitation goals.
• Educate patients and their families about treatment plans and self-care techniques.
• Collaborate with physical therapists, occupational therapists, and other rehabilitation professionals to optimize patient outcomes.
• Document patient care activities accurately and maintain up-to-date medical records.
• Provide emotional support and encouragement to patients and their families.
• Participate in multidisciplinary team meetings to discuss patient progress and coordinate care.
• Ensure compliance with health and safety regulations and protocols.
• Assist in the discharge planning process to ensure continuity of care post-rehabilitation.
• Stay updated on best practices and advancements in rehabilitation nursing care.

Qualifications

• Valid and current Registered Nurse (RN) license in the state of practice
• Bachelor of Science in Nursing (BSN) preferred
• Minimum of 2 years of nursing experience in a rehabilitation or related setting
• Certification in Rehabilitation Nursing (CRRN) preferred or willingness to obtain
• Basic Life Support (BLS) certification required
• Advanced Cardiovascular Life Support (ACLS) certification preferred
• Strong knowledge of rehabilitation nursing principles and practices
• Proficiency in electronic medical records (EMR) systems
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
• Strong problem-solving and critical thinking skills
• Ability to lift and move patients, and perform physically demanding tasks
• Availability to work flexible shifts, including nights, weekends, and holidays as needed
