Job summary

SummitWest Care in Grand Junction, Colorado seeks a compassionate Physical Therapist Assistant (PTA) to provide in-home therapy under licensed PTs. This flexible, 12-week contract offers independence and meaningful patient relationships in patients' own homes.

You will implement treatment plans, help improve mobility and safety, and document progress in EMR while coordinating with the broader care team.

Qualifications

• Current Colorado PTA license in good standing.
• Graduate of an accredited PTA program.
• Valid driver’s license and reliable transportation.
• Strong communication and documentation skills.
• Ability to work independently in a home-based setting.

Responsibilities

• Provide skilled therapy services in patient homes under supervision of PTs.
• Implement treatment plans with therapeutic exercise, gait, balance, mobility training.
• Monitor and report patient progress to supervising PT.
• Encourage patient participation and reinforce goals.
• Complete timely documentation in EMR.
• Collaborate with therapists, nurses, caregivers and care team.

We're seeking a compassionate, motivated Physical Therapist Assistant (PTA) to provide in-home therapy services under the direction of our licensed Physical Therapists. This position offers flexibility, independence, and the opportunity to build meaningful relationships with patients while helping them improve mobility, safety, and quality of life in their own homes.

Responsibilities

• Provide skilled therapy services in patient homes
• Carry out established treatment plans involving therapeutic exercise, gait training, balance activities, and functional mobility intervention
• Monitor and communicate patient progress and changes to supervising PT
• Encourage patient participation and reinforce goals
• Complete timely, accurate documentation in EMR
• Collaborate with therapists, nurses, caregivers, and other care team members

Requirements

• Current Colorado Physical Therapist Assistant (PTA) license in good standing
• Graduate of an accredited Physical Therapist Assistant program
• Valid driver's license, reliable transportation, and auto insurance
• Strong communication and documentation skills
• Ability to work independently and adapt professionally in home-based setting
