One East / West is a forty-bed unit specializing in stroke rehabilitation. Our team includes physicians board-certified in stroke rehabilitation, nurses, therapists, nursing assistants, psychologists, dietitians, and chaplains. Marianjoy Stroke Program is certified by the Joint Commission in Disease-Specific Care in Stroke Rehabilitation. The multidisciplinary team creates individualized treatment plans to manage physical, emotional, and behavioral changes, aiming to return patients to their families and communities. This setting offers opportunities for clinical skill development and professional growth.

Responsibilities

• Answer and triage patient call lights and meet care needs promptly.
• Assist with personal hygiene and activities of daily living to promote independence.
• Assist with application of assistive devices and prosthetics/orthoses.
• Perform skin care and report any changes immediately.
• Calculate and document calorie intake, output, and patient weights accurately.
• Assist with transfers and mobility using proper techniques and assistive devices.
• Support bladder and bowel programs, including specimen collection and catheterization under RN supervision.
• Record vital signs and communicate concerns to RN.
• Measure blood glucose levels and perform 12-lead EKGs as ordered.
• Assist with meal service and patient engagement activities.
• Monitor high-risk patients and provide de-escalation as needed.
• Participate in quality improvement activities and maintain unit readiness.
• Communicate effectively with patients and staff, maintaining therapeutic relationships.
• Contribute to infection control and safety protocols.
• Rotate across units based on patient care needs and adapt to policy changes.
• Serve as a preceptor and support new staff orientation.

Qualifications

Required:

• High school diploma or equivalent (GED)
• Current CNA certification or enrollment in RN program with two clinical rotations completed
• Current CPR certification
• Basic math skills
• Effective communication skills and customer service orientation
• Support of Northwestern Medicine’s Mission and Values
• Flexibility and adaptability to change
• 12-lead EKG competency within 6 months of hire

Preferred:

• 2-5 years of experience in rehab, acute care, or skilled nursing facilities
